Payment Policy

There are a variety of options for payment. Payments can me made weekly, bi-weekly, or monthly. If you prefer to pay monthly, we require that you pay a month in advance. Services will be suspended if payments are not made in a timely fashion. Please notify me as to the option that works best for you.

Cancellation Policy

In the event of a late cancellation or “no shows” the time scheduled for your child is not available to anyone else. Therefore, the following cancellation and “no show” policies must be observed.

The cancellation of a scheduled appointment must be made with a minimum of 24 hours notice. Any appointment cancelled with less than 24 hours notice will be billed a cancellation fee equal to the full rate of the scheduled session. In the event of illness, Mozaic Mindz must be notified no later than 10:00 AM the day of the scheduled appointment to avoid the cancellation fee. Policies for Schools

Mozaic Mindz provides contracted services to schools for professional development and Child Study Team services.  Mozaic Mindz will provide copies of fingerprints, certifications, registered business number, insurance, and sample contracts to the local school board.  Upon completion of services, invoices will be provided to the board for payment within 30 days.
